Different Kinds of Surgical Sperm Retrieval Methods
Surgical sperm retrieval is yet another aspect of assisted reproductive technology that allows aspiring couples to reach the point of parenthood. Surgical sperm retrieval, as a medical procedure, basically means taking out sperm from the testicles of the male partner by employing surgical methods, which can later be used to help couples attain pregnancy in combination with other ART procedures.
There are different kinds of surgical sperm retrieval methods that doctors use in accordance with the prerequisites of the case. Based on the medical condition the patient is troubled with, a fertility expert doctor might recommend:
- TESA: Testicular Sperm Aspiartion
- PESA: Percutaneous Epididymal Sperm Aspiration
- MESA: Microsurgical Epididymal Sperm Aspiration
- Micro-TESE: Microsurgical Testicular Sperm Extraction

Testicular Sperm Aspiration (TESA)
TESA, also called Testicular Fine Needle Aspiration (TFNA), is a procedure done to treat azoospermia by using a small needle to puncture the scrotal sack and the testis. The sperm is gently pulled out of the testis. This method of surgical sperm retrieval does not require any other incisions to be made.
Percutaneous Epidydmal Sperm Aspiration (PESA)
PESA surgery is another method of surgical sperm retrieval that is very similar to TESA. It does not require any imaging using a high-powered microscope. It can easily be carried out by a trained urologist under local or general anesthesia. However, the seminal fluid containing the sperm may not always come out. In that case, a surgical procedure may be recommended.
Microsurgical Epidydmal Sperm Aspiration
MESA is a complex procedure that requires it to be done under general anesthesia by a highly skilled microsurgeon. The procedure is done using a surgical microscope to retrieve sperm from the epidydmis that can later be cryopreserved and used to attain pregnancy.
Microsurgical Testicular Sperm Extraction
Commonly known as micro-TESE surgery, the procedure is only recommended for men with unbostructive azoospermia. Although micro-TESE surgery is more invasive than the other methods mentioned above, it is also believed to be more effective in locating and retrieving sperm. The process includes surgically opening the testes, providing a better view of the sperm-producing areas of the testes. There is also a lower chance of tissue or nerve damage with this method of surgical sperm retrieval.
